29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  391 St. Jude Industrial Court, Section 136: Employer is exposing employees to hazardous conditions where employees are permitted to work where there is accumulated dust on workplace floor. On 07/15/2021, employees were exposed to slipping hazards where they worked on walking-working surfaces where there were pronounced dust accumulations.  The accumulated dusts on some surfaces were measured at approximately 1 1/2 inches.

29 CFR  1910.178(p)(1): Powered industrial trucks found to be in need of repairs, defective, or in any way unsafe were not taken out of service until restored to safe operating condition(s):  391 St. Jude Industrial Park, Section 136: Employer is exposing employees to hazardous conditions where the employees are permitted to operate powered industrial trucks which have defects. On and about 07/15/2021, employees operated powered industrial equipment which had defects such as, but not limited to, missing anti-skid pads on brake pedals, damaged tires, and damaged headlights.  The trucks were not removed from service until they were repaired exposing the employees to struck- by hazards.

29 CFR  1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:   391 St. Jude Industrial Park, Marston, MO 63866, Section 136: Employer is exposing employees to hazardous conditions where the employees are permitted to use portable fans which have blade peripheries under 7 feet from workplace floor with larger than 1/2 inch openings in the guard.  On and about 07/15/2021, employees were exposed to amputation hazards where the guards of the blades of portable fans had openings measured at approximately one and one half (1 1/2) and five eighths (5/8) inches.  The peripheries of the metal blades of the portable fans were measured at less than five feet from workplace floor.

29 CFR  1910.157(c)(1): The employer did not mount portable fire extinguishers so that they were readily accessible to employees without subjecting the employees to possible injury:  391 St. Jude Industrial Park, Marston, MO  63866: Employer is exposing employees to tripping and fire hazards where the employees work in areas where portable fire extinguishers are left unmounted on workplace floors.  On and about 07/15/2021, employees were exposed to hazards of unmounted portable fire extinguishers.  Instances of the violation are:                               a) Section 136 where a portable fire extinguisher was left on its side on a concrete platform.                              b)  Pot Room where a portable fire extinguisher was left on its side on a concrete platform.  Magnitude 7 Metals LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1439121, citation number 1, item number1(a) and was affirmed as a final order on 04/20/2020, with respect to a workplace located at 391 St. Jude Industrial Park, Marston MO 63866.
