29 CFR  1926.501(b)(6):Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:  a) Buckley Ironworks, LLC located at 61 West Washington Street, Nelsonville, OH 45764:  On and preceding July 16, 2021, the employer did not ensure that employees were protected from falls when utilizing a plank for a walkway over a floor opening to a basement.  The plank walkway was in place due to the floor being demoed above the basement which was up to 8 feet above the lower level.  The following condition exposed employees to falls up to 8 feet.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) Buckley Ironworks, LLC located at 61 West Washington Street, Nelsonville, OH 45764:  On and preceding July 16, 2021, the employer did not ensure that the side rails of a ladder used to access/egress a basement extended at least 3 feet above the upper landing surface being accessed and /or the ladder was secured at its top.  A folded 8 foot self-supporting ladder being used for access/egress to a basement onto a plank extended approximately 2 inches beyond the upper landing being accessed.  The following condition exposed employees to falls up to 8 feet.

A folded 8 foot self-supporting ladder was being used for access/egress to a basement from/to a plank.  The following condition exposed employees to falls up to 8 feet.
