29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) On or about 7/13/2021 and at times prior, 901 2nd Court West, Birmingham, Al 35204, on the roof, employees replacing steel roofing plates on the roof of a steel framed warehouse were exposed to falls up to 24 feet from the roof to the concrete floor inside the building because protective equipment with defective components was not removed from service.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: (a) On or about 7/13/2021 and at times prior, 901 2nd Court West, Birmingham, AL 35204, on the roof, employees replacing steel roofing plates on the roof of a steel framed warehouse were exposed to falls up to 24 feet from the roof to the concrete floor inside the building because the employer did not provide adequate training in the correct procedures for inspecting the fall protection systems to be used.
