29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about June 8, 2021, at the winder deck, employees engaged in paper manufacturing were exposed to slip and fall hazards in that scraps of excess paper accumulated on the walking/working surfaces around the winder deck work area.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 8, 2021, at the winder deck, employees manually rolling paper were exposed to struck-by hazards in that rolls of paper were still in motion after hitting the second brake stop.
