29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  At the jobsite, employees were not wearing hardhats while working below other employees that were installing roofing material to a residential house, exposing employees to struck-by hazards from falling objects.

At the jobsite, employees operating Hitachi nail guns while performing roofing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.  Audelio Tinoco Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(01), which was contained in OSHA inspection number 1492762, Citation Number 1, Item number 1, and was affirmed as a final order on October 17, 2020, with respect to a workplace located at 7501 Daycoa Fort Worth, Texas 76120.

Audelio Tinoco Roofing, was previously cited for a REPEAT violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1528985, Citation Number 2, Item number 1, and was affirmed as a final order on June 14, 2021, with respect to a workplace located at 1274 Lamar Drive, Mansfield, Texas 76034.  Audelio Tinoco Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1492762, Citation Number 1, Item number 2, and was affirmed as a final order on October 16, 2020, with respect to a workplace located at 17504 Daycoa Street Fort Worth, TX 76120

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
