29 CFR 1910.147(c)(2)(i): Where an energy isolating device was not capable of being locked out, the employer's energy control program under 29 CFR 1910.147(c)(1) did not utilize a tagout system: a) On or about July 16, 2021, the employer failed to ensure that employees were protected from struck by, caught in between and electrical hazards when changing out the blade on the saw machine ( Amada Saw, serial #10110015). Saw machine operators change the saw blades out at least once per shift and do not lock and tag out the machine which has energy sources such as electrical and pneumatic. b) On or about July 16, 2021, the employer failed to ensure that employees were protected from struck by, caught in between and electrical hazards when changing out the blade on the saw machine (Promacut Saw, serial # GF598). Saw machine operators change the saw blades out at least once per shift and do not lock and tag out the machine which has energy sources such as electrical and pneumatic. c) On or about July 16, 2021, the employer failed to ensure that employees were protected from struck by, caught in between and electrical hazards when changing out the blade on the saw machine (Trennjaeger Saw, serial #GD269). 29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) On or about July 16, 2021, the employer failed to ensure that employees who were tasked with performing saw machine blade change outs were trained as authorized to perform lock out tag out and were applying lock out tag out prior to performing this task.
