The following condition exposed sub-contracted workers to falls exceeding 40 feet from balconies, window openings and scaffolds and struck-by hazards when working on and around scaffolding.

The fourth tier and other sections of the scaffolding did not have guardrails or other means of fall protection installed which exposed employees to falls in excess of 40 feet at the upper tiers of the scaffolding.

The following condition exposed employees to struck-by hazards from tools and materials falling from the scaffolding.
