29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) (a) At a jobsite located at 1601 SW State Rte 150 Lee's Summit, MO, employees were exposed to trip hazards when the employer failed to ensure all working levels of a scaffold, facing the north side of the building, was not fully planked. Employees were using the scaffold to access the side of the building to conduct stone siding work. (b) At a jobsite located at 1601 SW State Rte 150 Lee's Summit, MO, employees were exposed to trip hazards when the employer failed to ensure all working levels of a scaffold, facing the east side of the building, was not fully planked. Employees were using the scaffold to access the side of the building to conduct stone siding work.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) At a jobsite located at 1601 SW State Rte 150 Lee's Summit, MO, an employee was exposed to fall hazards when the employer failed to ensure the employee was protected from falling from a distance of greater than 6 feet. An employee was observed working on a roof ledge cutting and applying stone to the side of the building without any form of fall protection.
