29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   On or about May 11, 2021, the employer did not ensure that each employee engaged in servicing and maintenance work was trained to recognize and control hazardous energy sources prior to starting work.  An employee who was cleaning the blades on a large overhead fan relied on another employee to lockout out the fan prior to beginning work because they had not been trained on how to lockout the unit.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  On or about May 11, 2021, the employer did not ensure that each employee who was engaged in servicing and maintenance work applied their own lock to each energy isolation device.  An employee who was cleaning the blades on a large overhead fan had relied on another employee to lock out out the fan prior to beginning work.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard:  On or about June 23, 2021, the employer did not ensure that the #505 200 ton Minster part revolution mechanical power press's point of operation was guarded to protected employees from crushing and amputation injuries.  The guard on the backside of the press had an opening large enough to allow an employee to reach through and into the point of operations.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
