29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about April 23, the employer did not ensure that employees who engaged in set-up and troubleshooting activities on the Brutus 4,400 Ton Heavy Gauge Stamping Press received training in the recognition of applicable hazardous energy sources and the methods and means necessary for isolation and control.  Employees were exposed to the potential for unexpected start-up, energization, and/or movement of the equipment, exposing them to struck-by and caught-between hazards from press dies and the transfer system arms.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about April 23, 2025, the employer did not ensure that employees implemented energy control steps prior to performing servicing and maintenance activities on the Brutus 4,400 Ton Heavy Gauge Stamping Press.  Hazardous energy was not isolated prior to employees troubleshooting and setting up the equipment, exposing them to struck-by and caught-between hazards from press dies and transfer system arms.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.   a)  On or about April 23, 2025, the employer did not conduct and properly document periodic inspections of the company's energy control procedures as required.  The employer's hazardous energy control program had deficiencies including, but not limited to employees not locking out machines or equipment prior to servicing and maintenance activities.  These deficiencies could have been identified and corrected if the inspections were conducted and documented as required.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
