On or about July 23, 2021, during a plug connection on the west side of Sprague road, near the intersection of Sprague and Thornhurst in North Royalton, Ohio, the employer did not ensure an employee working inside a trench, with a depth of approximately 6 feet 7 inches, and approximately 90 degree vertical walls, was protected rom cave-ins by an adequately designed protection system. The soil was classified as; "hard-clay", Cohesive Soil, Type "B".
