29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a) On or about November 5, 2024, during the installation of an eight-inch medium pressure gas lines at 10007 Shepard Road, Macedonia, Ohio 44056, the employer did not ensure employees working inside a trench, with a depth of approximately eight-feet, and approximately 90 degree vertical walls, was protected from cave-ins by an adequately designed protection system. The soil was classified as; pre-disturbed, Type "B".  The Vallejo Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 1543420, citation number 1, item number 1, and was affirmed as a final order on August 26, 2021, with respect to a workplace located at 4485 Sprague, Parma, Ohio 44134.
