29 CFR 1926.502(d)(6)(i): A snap hook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snap hook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snap hook such that unintentional disengagement could occur by the connected object being able to depress the snap hook keeper and release itself. (a) On or about July 23, 2021, at the construction site located at 536 Main St, Allenton WI, employees were exposed to a fall hazard of approximately 21 8/10 feet and had the snap hook connected directly to webbing of the vertical life line.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about July 23, 2021, at the construction site located at 536 Main St, Allenton WI, an employee was tied off with a harness and vertical lifeline and fell through the roof opening where they were replacing roof sections. The employee fell 21 8/10 feet and hit the ground. The employee free fell more than 6 vertical feet.
