29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On the 5:12 pitch roof, at the front of the house, at 219 Pepperpike Way in Saint Augustine, Florida: On or about July 20, 2021, the employer exposed three framing employees to a 10 feet fall hazard at the roof eave in that the roof access ladder rails were not extended 3 feet above the upper landing surface of the roof when they accessed the roof to install sheathing boards.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. Employees performing roof sheathing are not protected from fall hazards by the use of a fall protection system. Father N Sons Construction, LLC fails to assure effective protections against falls as required by the standard, as demonstrated on the 5:12 pitch roof, on all slopes of the roof, at 219 Pepperpike Way in Saint Augustine, Florida: On or about July 20, 2021, the employer exposed three framing employees to a 10 feet fall hazard from the roof eaves and a fall hazard of up to 18 feet through the trusses while installing roof sheathing boards in that they were not protected by a fall protection system. Father N Son Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1301214, Serious citation number 1, item number 1 and was affirmed as a final order on September 14, 2021, with respect to a workplace located at 119 Palisade Drive, Saint Augustine, Florida.
