29 CFR 1910.94(a)(3)(i)(c): Before the enclosure was opened, the blast exhaust system was not run for a sufficient period of time to remove the dusty air within the enclosure.   a) 3350 Elm Ave, Portsmouth VA 23704; Onsite blasting facility: On or around 8/3/21, the employer did not allow the abrasive blasting exhaust system to run for a sufficient period of time to remove the dust contaminated air within the blasting enclosure, following the abrasive blasting of production material. This resulted in the observation of a large visible cloud of abrasive blasting dust being released into the general work environment where various employees were working without respiratory protection.   Corrected during the inspection.

Dust shall not be permitted to accumulate on the floor or on ledges outside of an abrasive-blasting enclosure, and dust spills shall be cleaned up promptly. Aisles and walkways shall be kept clear of steel shot or similar abrasive which may create a slipping hazard;    (a) 3350 Elm Ave, Portsmouth VA 23704; Onsite blasting facility: On or around 8/3/21, dust was not cleaned up promptly producing a significant accumulation on the floor, ledges, aisles and walkways of the abrasive blasting facility, which created a slipping hazard.    Corrected during the inspection.

29 CFR 1915.152(b):The employer did not assess its work activity to determine whether there were hazards present, which necessitate the employee's use of personal protective equipment.   a) 3350 Elm Ave, Portsmouth VA 23704: On or around 8/3/21, the employer did not assess, by means of a quantitative personal air sampling exposure assessment, the inhalation hazards produced during brush & roll and spray painting operations of epoxy coatings containing hazardous volatile organic compounds, in order to assure that the respiratory protection requirements are adequate in reducing employee exposures below the occupational exposure limit.    b) 3350 Elm Ave, Portsmouth VA 23704: On or around 8/3/21, the employer did not assess, by means of a quantitative personal air sampling exposure assessment, the inhalation hazards produced during abrasive blasting operations, in order to assure that the respiratory protection requirements are adequate in reducing employee exposures below the occupational exposure limit.  Abatement required within 10 days after abatement date.
