29 CFR 1915.87(e): The employer did not ensure that quick-drenching and flushing of the eyes and body were located for immediate emergency use and within close proximity to the operations where such substances were being used. a) 3350 Elm Ave, Portsmouth, VA, paint application building: On 8/3/21, CSHO observed a designated eyewash station in the painting work area, where the application of two part epoxy coatings (Polysiloxane XLE-80 HF Semi-gloss Parts A and B) occur on a frequent basis. The eyewash station for this area was missing the flushing solution required for operation. Corrected during the inspection.

29 CFR 1915.507(b)(1):The portable fire extinguishers and hose systems were not selected, installed, inspected, maintained, and tested by the employer according to NFPA 10-2002 Standard for Portable Fire Extinguishers. a) 3350 Elm Ave, Portsmouth, VA: On 8/3/21, CSHO observed a fire extinguisher on the Kalmar DCD 250 forklift being operated by employees. The extinguisher's inspection tag's last recorded inspection was April of 2021. Corrected during the inspection.
