29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: (a) In the Palletizer of Line 4- On or about July 28, 2021 and at times prior and thereafter the energy control procedure (SHE #S009) failed to clearly identify all of the specific steps to be followed by employees to isolate and disable the palletizer prior to employees trying to enter the palletizer to correct leaning rolls. The procedures also lacked specific verification steps to determine the effectiveness of the energy control measures.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Palletizer of Line 4- On or about July 28, 2021 and at times prior and thereafter, the employer failed to shut down or turn off the machine to perform servicing in accordance with their procedure, thereby exposing employees to crush by hazards while they serviced the palletizer.
