29 CFR  1910.29(b)(1): The employer did not ensure that  guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11 of this section).  a) Line 4, Mixer Platform (left side) - On or about November 17, 2021, the employer did not ensure that the top edge height of the chain used as the top rail on the rear loading side of the platform was 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The lowest point of the chain was approximately 32 inches above the platform's walking-working surface, which exposed the effected employee to a fall hazard of approximately 6 feet.  b) Line 4, Mixer Platform (right side) - On or about November 17, 2021, the employer did not ensure that the top edge height of the chain used as the top rail on the rear loading side of the platform was 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface.  The lowest point of the chain was approximately 33 inches above the platform's walking-working surface, which exposed the effected employee to a fall hazard of approximately 6 feet.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails are installed at a height midway between the top edge of the guardrail system and the walking-working surface.   a) Line 4 - Mixer Platform (left side) - On or about November 17, 2021, the employer did not ensure that a midrail was installed on the rear loading side of the platform.  The effected employee was, thereby, exposed to a fall hazard of approximately 6 feet.  b) Line 4 - Mixer Platform (right side) - On or about November 17, 2021, the employer did not ensure that a midrail was installed on the rear loading side of the platform.  The effected employee was, thereby, exposed to a fall hazard of approximately 6 feet.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Line 1 Conveyor, Near Proofer Exit - On or about November 16, 2021, the employer did not ensure effective machine guarding was provided to protect employees working at or near a rotating conveyor belt roller where the conveyor belt transitions from horizontal to vertical at the beginning of the conveyor section, thereby exposing employees to hazards associated with ingoing nip points and rotating parts.  b) Line 2, Metal Detector Area - On or about November 15, 2021, the employer did not ensure effective machine guarding was provided to protect employees working at or near a rotating conveyor belt roller located on the underside of the conveyor just prior to the conveyor's transition to the metal detector section, thereby exposing employees to hazards associated with ingoing nip points and rotating parts.  c) Line 2, Stripping Room - On or about November 15, 2021, the employer did not ensure effective machine guarding was provided to protect employees working at or near rotating conveyor belt rollers located on the bottom of each cheese waterfall conveyor, thereby exposing employees to hazards associated with ingoing nip points and rotating parts.

29 CFR  1910.147(c)(4)(i):  Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) Line 2, Metal Detector - On or about August 24, 2021, the employer did not ensure that an employee utilized lockout procedures (LP1147) while engaged in the service and/or maintenance task of drying the conveyor belt.   Rich Products Corporation was previously cited for a violation of this occupational safety and health standard  29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1218355, citation number 1, item number 1 and was affirmed as a final order on August 3, 2017, with respect to a workplace located at 4600 Northwest Parkway, Hilliard, Ohio 43026.
