29 CFR  1910.29(b)(10): When guardrail systems are used at hoist areas, a removable guardrail section, consisting of a top rail and midrail, are placed across the access opening between guardrail sections when employees are not performing hoisting operations.  The employer may use chains or gates instead of a removable guardrail section at hoist areas if the employer demonstrates the chains or gates provide a level of safety equivalent to guardrails.  The employer did not ensure that the removable guardrail for the Press 4 Pit consisted of a top rail and a midrail across the access opening used for hoisting operations, exposing employees to fall hazards. The depth of the Press 4 Pit was measured to be greater than four feet.

29 CFR  1910.29(b)(13)(i): When guardrail systems are used around holes that serve as points of access (such as ladderways), the guardrail system opening shall have a self-closing gate that slides or swings away from the hole, and is equipped with a top rail and midrail or equivalent intermediate member that meets the requirements in paragraph (b) of this section.  The employer did not ensure that the self-closing gate that was used for access into the Press 4 Pit consisted of a top rail and midrail across the opening, exposing employees to fall hazards. The depth of the Press 4 Pit was measured to be greater than four feet.

29 CFR  1910.29(k)(1)(i):(i) The employers must ensure toeboards used for falling object protection are erected along the exposed edge of the overhead walking-working surface for a length that is sufficient to protect employees below.  The employer did not ensure that toeboards were installed along the overhead edge of the Press 4 Pit to protect employees working below, exposing employees to struck by falling object hazards. The depth of the Press 4 Pit was measured to be greater than four feet.

29 CFR  1910.147(d)(3):Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  a) On or about July 28, 2021, the employer did not ensure that an employee performing maintenance tasks, including but not limited to removing a flare-up on the Press 4 Extrusion Press  (Contract Number DP0KGG01), had isolated all energy sources, including but not limited to electrical energy, exposing employees to hazardous energies. A keyed emergency stop pushbutton (Model 3SB3500-1BA20) was locked and used in place of an energy isolating device while performing the work. b) On or about August 3, 2021, the employer did not ensure that an employee performing tasks, including but not limited to cleaning and lubricating on the Press 4 Scrap Conveyor, had isolated all energy sources, including but not limited to electrical energy, exposing the employee to hazardous energies. An emergency stop pushbutton (Model ZB4BT84) with a padlockable guard (Model ZBZ1605) was locked and used in place of an energy isolating device while performing the work.
