29 CFR  1904.40(a) Reference standard 1960.66(a):The employer did not provide an authorized government representative the records within the four business hours.   JOHN F KENNEDY DAY SCHOOL: On or about August 4, 2021, the employer failed to provide copies of the requested injury and illness records for calendar years 2018, 2019, 2020 and 2021 to an authorized OSHA representative when requested both verbally and in writing, as the employer had not kept records.   The Bureau of Indian Education was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1960.66(b), which was contained in OSHA inspection number 1502674, citation number 2, item number 2, issued on January 19, 2021, which was affirmed as a final order on March 12, 2012 with respect to a workplace located at Navajo Rte. 27, 18 mi N. of Ganado, Ganado, AZ, 86505. Abatement Certification and Documentation is required
