29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  a)  On August 10, 2021, employees working from frame scaffolding were exposed to fall hazards of approximately 21 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one stairrail system along each unprotected side or edge:  a)  On August 10, 2021, employees were accessing the upper floors of the building by way of stairways that did not have a stairrail system along the unprotected side.
