29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. On or about August 19, 2022, at the above addressed jobsite, the employer failed to ensure that an employee required to operate a Genie GTH-1056 Telehandler was adequately trained as demonstrated by the successful completion of a training and evaluation program. Phoenix Masonry, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1582509, citation number 1, item number 1, and was affirmed as a final order on June 29, 2022, with respect to a workplace located at 2215 N. Halsted St. in Chicago, Illinois.
