29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: a) On or about August 12, 2021 the in-running pinch point hazard created by the metal belt and the end rollers on the interior corner of the transfer belt on the outfeed oven conveyor system was not guarded to prevent employee exposure. b) On or about August 12, 2021 the in-running pinch point hazard created by the metal belt and the end rollers on the exterior corner of the transfer belt on the outfeed oven conveyor system was not guarded to prevent employee exposure.

Sprockets and V-belt drives located within reach from platforms or passageways or located within 8 feet 6 inches from the floor shall be completely enclosed: a) On or about August 12, 2021, on the outfeed oven conveyor an in-running pinch point hazard created by a sprocket and belt chain conveyor adjacent to the corner of a main walk-way and the east exterior wall was not guarded to prevent employee exposure.

29 CFR 1910.219(c)(4)(i):Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves: On or about August 12, 2021, the length of the shaft protruding from the powered end of the metal belt conveyor at the outfeed section of the oven in the Hot Press room was not less than one-half the diameter of the shaft, exposing employees to a potential entanglement hazard.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about August 12, 2021 the front solid metal hinged guard on the Apache Toro 2048 was not affixed closed to the machine frame and did not prevent employee access to the downward motion of the platen. An interlock was affixed to the guard was not functioning.
