29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On August 12, 2021, employees utilized a ladder to access the roof of a townhome and the ladder's side rails did not extend 3 feet above the landing. This exposed employees to fall hazards of ~16 feet.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). (a) On August 12, 2021, employees utilized a ladder to access the roof of a townhome and ladder was placed at an angel of ~47degrees ( ~7:15 ratio). This exposed employees to fall hazards of ~16 feet.
