29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 12, 2021, at the jobsite, the employer did not ensure employees were in use of fall protection while working from an aerial lift installing trimmings to a building. This exposed the employees to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 12, 2021, at the jobsite, the employer did not ensure that an employee, performing framing work activities while standing on the top-plate of a building frame, was in use of fall protection. This exposed the employee to a fall hazard greater than six(6) feet to the ground below.
