29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)	The employer failed to provide documentation and certification demonstrating the abatement method and completion for an employee using a ladder improperly to access a roof issued on August 13, 2022 as Citation 1 Item 1.  (b) The employer failed to provide documentation and certification demonstrating the abatement method and completion for an employee  exposed to a fall hazard while performing roofing work without fall protection issued on August 13, 2022 as Citation 2 Item 1.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about August 13, 2021, at a job site located at 2574 South Williams Street, Milwaukee, WI, 53207, employees used four (4) aluminum extension ladders to access the roof.  The ladder side rails for all four (4) extension ladders did not extend at least three (3) feet above the upper landing surface being accessed, exposing employees to an approximate 24 foot fall hazard.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about August 13, 2021, at a job site located at  2574 South Williams Street, Milwaukee, WI, 53207,  employees were engaged in tearing off shingles on an 8/12 pitched roof, were not protected from falling to the lower level.  This exposed employees to a fall hazard of approximately 24 feet.  The PBS Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1474486, citation number 1, item number 1 and was affirmed as a final order on June 8, 2020, with respect to a workplace located at 1893 Haymarket Road, Waukesha, WI 53189.
