29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack was not blocked: On or about August 10, 2021, Citadel Rig #6 location; the jacks at the front of the Directional Driller's trailer were not blocked on muddy cracked ground. This condition exposed employees to a crush-by hazard.

29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about August 10, 2021, Citadel Rig #6 location, the Directional Driller's trailer was not equipped with a mid-rail opening that measured at least 19 inches. This exposed employees to fall hazards.
