On or about August 16, 2021 and at times prior thereto, the employer did not ensure the scaffold was fully decked. Employees working from the platform installing masonry plastering were exposed to a fall hazard.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. On or about August 16, 2021 and at times prior thereto, the employer did not ensure toe boards were installed on a scaffold while employees worked from areas below the workers performing masonry work activities from the scaffold. This exposed the employees to struck by hazards.
