29 CFR  1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  The employer is failing to protect employees from slip and fall hazards while working at the Aida Press (Model PPMX-L2-400, Camaco machine PS69) and around the material transfer location near the Muller press. This was most recently evident during an inspection on August 17, 2021 at the Camaco, LLC facility located at 1851 E 32nd Ave, Columbus, NE 68601. The employer is exposing employees to slippery work conditions while working at Aida Press (Model PPMX-L2-400, Camaco machine PS69) and transferring lubricants/oil near the Muller press. The Aida press uses Klenedraw W-4888 as a lubricant and during operation, the press generates small amounts of spray onto the diamond metal floor causing a slippery working surface. While transferring lubricants/oils from 55-gallon barrels to 5-gallon buckets, employees are exposed to slippery walking-working surfaces from lubricants/oils spilling onto the floor.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  The employer is failing to protect employees from fire and electrical shock hazards by using a flexible extension cord to provide power from a relocatable power tap (power strip). This was most recently evident during an inspection on August 17, 2021 at the Camaco, LLC facility located at 1851 E 32nd Ave, Columbus, NE 68601. The employer is using a relocatable power tap that is series-connected to an extension cord instead of a permanently installed branch circuit receptacle. The power tap is being used in the employee entrance work area to provide power to a CribMaster Vending machine that dispenses gloves for employees.

29 CFR  1910.134(d)(1)(iii):The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.  Employer is failing to monitor respiratory hazards in areas including, but not limited to Area #190. This was most recently observed at Camaco, LLC, 1851 E. 32nd Ave., Columbus, Nebraska on August 18, 2021 where the employer had moved an automated welding machine (WR171) to a new location and did not connect the exhaust ventilation.
