29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  1024 Kitty Hawk: On or about August 18, 2021, and at times prior thereto, employees were exposed to a fall hazard to the ground below while working from a Skyjack SJIH 3219 Scissor Lift that had not been operated by a competent person.

29 CFR  1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section.  1024 Kitty Hawk: On or about August 18, 2021, and at times prior thereto, an employee was exposed to a fall hazard greater than 15  feet to the concrete floor while connecting steel beams in a commercial construction site without fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a-c) On or about August 18, 2021, and at times prior thereto, employees were exposed to a fall hazard while using 20-feet Werner extension ladders that were not extended three feet from the roof.
