29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Jacob A. Weaver on or about August 05, 2021 at the address of 30777 Guys Mills Rd., Townville, Pa: The notching machine was operated without proper machine guarding in place. Employees swing open a guard during normal operation, to allow the employee to clear a fallen board in the start-up process, exposing employees to moving parts hazards. An employee suffered a finger amputation.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). A) Jacob A. Weaver on or about August 05, 2021 at the address of 30777 Guys Mills Rd., Townville, Pa: An employee did not utilize lockout/tagout during servicing and maintenance of the machine. The employee did not lockout the machine before picking up a fallen board in and around the danger zone area. resulting in an amputation. An employee suffered a finger amputation.

29 CFR 1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: A) Jacob A. Weaver on or about August 19, 2021 at the address of 30777 Guys Mills Rd., Townville, Pa: The area in front of the electrical panel did not allow enough room for access or work space. The panel had a transfer table roughly 14 inches from the front of the panel . Employees use this transfer table to stack wood during normal operation. B) Jacob A. Weaver on or about August 19, 2021 at the address of 30777 Guys Mills Rd., Townville, Pa: The area in front of the electrical disconnect box did not allow enough room for access or work space. The panel had a transfer table roughly 14 inches from the front of the disconnect. Employees use this transfer table to stack wood during normal operation.
