29 CFR 1926.601(b)(4): The employer had in use motor vehicle equipment having an obstructed view to the rear which did not have a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to back up the vehicle. On/or about August 20, 2021, the employer had not ensured that the backup alarm and back up lights on the white 2017 C3500 Chevy Silverado utility pickup truck were operable. As a result of the inoperable backup alarm and back up lights, an employee that was emerging from the parked service truck area at the Benson Road/Southford Road, Middlebury, CT was struck by the utility pickup truck exposing the employee to head injuries. ABATED DURING THE INSPECTION.
