29 CFR 1926.62(d)(1)(i): The employer did not initially determine if any employee may be exposed to lead at or above the action level:  80 South Main Street Bridge No. 00454 - On or about June 22, 2023, and sometime thereafter, the employer did not initially determine if employee exposures to lead were at or above the action level while work operations, covered by 29 CFR 1926.62 (d)(2)(iv), were performed. Employees were required to conduct highway bridge renovation tasks that included, but were not limited to, the removal of paint by way of abrasive blasting and cutting operations.

29 CFR 1926.62(d)(1)(iii): The employer did not collect personal samples representative of a full shift, including at least one sample for each job classification in each work area, either for each shift or for the shift with the highest exposure level:  80 South Main Street Bridge No. 00454 - On or about June 22, 2023, and sometime thereafter, the employer did not perform an initial determination of exposures based on the relevant considerations of information and observations that indicated employees were exposed to lead. Operations performed that were covered by 29 CFR 1926.62 (d)(2)(iv) included, but were not limited to, abrasive blasting and cutting.

29 CFR 1926.62(d)(2)(v)(A): In the interim, until the employer performs an employee exposure assessment as required under 29 CFR 1926.62(d) and determines actual employee exposure, the employer did not provide to employees performing the tasks described in 29 CFR 1926.62(d)(2)(i)(A) with appropriate respiratory protection in accordance with 29 CFR 1926.62(f):  80 South Main Street Bridge No. 00454 - On or about June 22, 2023, and sometime thereafter, without having performed an employee exposure assessment, the employer failed to provide employees who performed tasks described in 29 CFR 1926.62(d)(2)(iv) with the appropriate respiratory protection in accordance with the provisions of 29 CFR 1926.62(f).

29 CFR 1926.62(f)(1)(iv): Appropriate respiratory protection was not used during periods when respirators were required to provide interim protection for employees while they performed the operations specified in 29 CFR 1926.62(d)(2):  80 South Main Street Bridge No. 00454 - On or about June 22, 2023, and sometime thereafter, the employer failed to provide employees with respiratory protection in accordance with the appropriate assigned protection factor (APF) for the presumed exposure in excess of 2,500 ?g/m3 that required a full-face respirator with an APF of at least 50 considering the 1926.62(d)(2)(i)(A) tasks performed. Employees wore and were fitted for elastomeric half-face air purifying respirators that had an APF of 10 at the time of this inspection.

29 CFR 1926.62(i)(5)(i): The employer did not provide adequate handwashing facilities in accordance with 29 CFR 1926.51(f), for use by employees exposed to lead:  80 South Main Street Bridge No. 00454, employees were not provided with adequate handwashing facilities in accordance with 29 CFR 1926.51(f) during tasks when lead-based painted steel bridge structures were abrasive blasted with equipment including, but not limited to, pneumatic bristle blasters (Part No. HTROTBPA) and chisel/needle scalers (Jet Chisel, Model JEX-24).
