29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures: On or about August 16, 2021 and times prior thereto, where employees were exposed to caught-in and amputation hazards when the employer did not provide updated and additional training when the SWECO XS60 separator changes were made such as but not limited to: a) the Dust Cover and the ACS rotary valve chute was removed for approximately 2 months; and b) the ACS rotary valve chute clogging issues arose due to increased water usage in the cleaning process of the cooling section.
