29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) Absolute Quality Roofing. On or about August 25, 2021, at the site location of 1108 White St. Toledo, OH: The employer did not insure flexible cords had working strain relief fittings in place to prevent energized wires from being pulled out. Employees working in this condition were exposed to electrical shock.

On or about August 25, 2021, at the site location of 1108 White St. Toledo, OH: The employer did not require the use of any form of fall protection during roofing activities, exposing employees to falls up to 19ft.

On or about August 25, 2021, at the site location of 1108 White St. Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing activities. Employees operating in this condition were exposed to falls up to 19ft.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Absolute Quality Roofing. On or about August 25, 2021, at the site location of 1108 White St. Toledo, OH: The employer did not ensure the side rails of the aluminum extension ladder used on site to access the roof extended at least three feet above the landing surface. Employees in this condition were exposed to falls of 13 ft.
