29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Absolute Quality Roofing LLC; on or about August 7, 2024, at the address of Windrush Bourne Bowling Green, OH 43402. The employer did not ensure that employees were protected from head injury hazards by protective hardhats while working on the ground below as employees above actively performed roofing activities. Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and building materials.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) Absolute Quality Roofing LLC; on or about August 7, 2024, at the address of Windrush Bourne Bowling Green, OH 43402. The employer did not ensure that employees were protected from eye injury hazards by protective eyewear while using pneumatic powered nail guns. Employees operating the powered nail guns in this condition were exposed to struck by injuries from ricochets and splintering building materials.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Absolute Quality Roofing LLC; on or about August 7, 2024, at the address of 76 Windrush Bourne Bowling Green, OH 43402: The employer did not ensure employees were protected from falls while performing residential roofing activities upon an 8/12 roof pitch  residential structure. Employees operating in these conditions were exposed to falls of 9' 5" at the eave to the level below.  Absolute Quality Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection # 1549540, citation number 1, item number 2 and was affirmed as a final order on 12/23/2021, with respect to a workplace located at 1108 White St. Toledo, OH 43605

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Absolute Quality Roofing LLC; on or about August 7, 2024, at the address of 76 Windrush Bourne Bowling Green, OH 43402: The employer did not ensure ladder rails were extended 3ft. above the landing surface where employees accessed the 8/12 pitch roof of a residential structure. Employees operating in these conditions were exposed to falls of 9' 5" at the eave to the lower level below.  Absolute Quality Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection #1549540, citation number 1, item number 4 and was affirmed as a final order on 12/23/2021, with respect to a workplace located at 1108 White St. Toledo, OH 43605
