29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: (a) TruckPro, LLC dba Midwest Truck Parts and Service at 5075 Cook Street, Denver, CO 80216: On and before August 25, 2021, abrasive wheels used on a DeWalt 4 1/2" DWE402 angle grinder, in the garage, were not provided with a safety guard. This condition exposed employees to the hazard of being struck by shrapnel in the event of a grinding wheel failure.

29 CFR 1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) TruckPro, LLC dba Midwest Truck Parts and Service at 5075 Cook Street, Denver, CO 80216: On and before August 25, 2021, the employer did not ensure all required screws were installed to secure the cover plate to an electrical outlet box that supplied power for a Milwaukee chop saw, a space heater, and a Sunex Tools drill press. This condition exposed employees to the hazard of electrical shock.

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) TruckPro, LLC dba Midwest Truck Parts and Service at 5075 Cook Street, Denver, CO 80216: On and before August 25, 2021, the employer did not ensure visual inspections of flexible cord sets and plug-connected equipment for external defects and for evidence of possible internal damage before employee use. An extension cord located in the garage of the facility had a damaged outer jacket. This condition exposed the employees to electrical shock hazards. (b) TruckPro, LLC dba Midwest Truck Parts and Service at 5075 Cook Street, Denver, CO 80216: On and before August 25, 2021, the employer did not ensure visual inspections of flexible cord sets and plug-connected equipment for external defects and for evidence of possible internal damage before employee use. A cord supplying power to a table mounted utility outlet box in the workshop had a damaged outer jacket. The utility box supplied power for a Milwaukee chop saw, a space heater, and a Sunex Tools drill press. This condition exposed employees to electrical shock hazards.
