Condition observed on or about 08/25/21. IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a- 19 Marshall Way, Tabernacle NJ - A worker was exposed to fall hazards, in excess of 21 feet to the ground surface below, when performing siding installation and other related activities from a pump jack scaffold that contained a mid rail/screen that did not run the full length of the scaffold's active walking/working surface. Condition observed on or about 08/25/21. OAP Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 1516029 citation number 1, item number 1 and was affirmed as a final order on 07/26/21, with respect to a workplace located at 11 Grande Boulevard, Tabernacle NJ. IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
