Each employee who was engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. At Lot 134 Grey Wolf DR, Imperial, MO, 63052 - On or about August 18, 2021, employees of Kyrios INC were exposed to elevated fall hazards of approximately 8-11 feet while working on an elevated unprotected edge of a residential structure and were not protected by a guardrail systems, safety net system, or personal fall arrest system.

At Lot 134 Grey Wolf DR, Imperial, MO 63052 - On or about August 18, 2021 employees of Kyrios Inc were exposed to possible fall hazards from a ladder used to access an upper landing surface which did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.
