29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities six feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about 10/27,2022, Kyrios, Inc. was installing roofing materials on a residential building located at 271 Wolfpack Dr. in Imperial, MO.  Four employees were observed on a 10/12 sloped roof, approximately 13-25 feet above the ground without utilizing fall protection.  Kyrios, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1558387, citation number 1, item number 1 and was affirmed as a final order on 10/12/2021, with respect to a workplace located on lot 143 Grey Wolf Dr. in Imperial MO.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least three feet (.9 m) above the upper landing surface.  On or about 10/27,2022, Kyrios, Inc. was installing roofing materials on a residential building located at 271 Wolf Pack Dr. in Imperial, MO. The extension ladder the employees were using to access and egress the roof did not extend three feet above the landing surface or edge of the roof.  Kyrios, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1558387, citation number 1, item number 2 and was affirmed as a final order on 10/12/2021, with respect to a workplace located on lot 143 Grey Wolf Dr. in Imperial MO.

29 CFR 1926.1053(b)(5)(i):  Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  On or about 10/27,2022, Kyrios, Inc. was installing roofing materials on a residential building located at 271 Wolfpack Dr. in Imperial MO.  Four employees were observed using an aluminum extension ladder to access and egress of the roof.  Employees were using the extension ladder at  an angle such that the horizontal distance from the top support to the foot of the ladder (approximately  21 feet) is more than one-quarter of the working length of the ladder (15 feet approximately).
