29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. On or about August 13, 2021, employee(s) were assigned tasks that would require lock out tag out such as cleaning and changing rollers while operating the Base Coater and the employer did not ensure that lockout/tagout training, was provided to employees to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about August 13, 2021 an employee was operating a base coater during the operation employee(s) wipe down the rolls on the base coater daily and change the rolls to a different size periodically. The operator(s) were exposed to amputation and crushing hazards to hands and fingers from unexpected startup while cleaning and changing the rolls. Employees were not required to control hazardous energy sources in the following sequence. 1) turn off and shut down the machines, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy, 5) verify that isolation and de-energization of the machines or equipment have been accomplished.
