29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: a) On or about August 24, 2021, the employer failed to ensure that there was guarding in place on the puller line #10. There are pinch points where the rollers pull the extruded plastic hoses into the machine for stretching and there is no guard in place. Employees have direct access to the ingoing nip points at the front of the puller line where the rollers have less than a 1.5 inch gap between them. Employees are exposed to caught in between hazards. b) On or about August 24, 2021, the employer failed to ensure that there was guarding in place on the puller line #16. There are pinch points where the rollers pull the extruded plastic hoses into the machine for stretching and there is no guard in place. Employees have direct access to the ingoing nip points at the front of the puller line where the rollers have less than a 1.5 inch gap between them. Employees are exposed to caught in between hazards. c) On or about August 24, 2021, the employer failed to ensure that there was guarding in place on the puller line #18. There are pinch points where the rollers pull the extruded plastic hoses into the machine for stretching and there is no guard in place. Employees have direct access to the ingoing nip points at the front of the puller line where the rollers have less than a 1.5 inch gap between them. Employees are exposed to caught in between hazards. d) On or about August 24, 2021, the employer failed to ensure that there was guarding in place on the splicer machine next to the puller line #16. The splicer forming machine is operated without continuous pressure or two hand controls and is activated with the push of one button. The splicer forming machine bends the plastic parts. puller line #10. Employees are exposed at the point of operation to struck by and caught in between hazards. NOTE: The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1381977, citation 1, item 1, which was affirmed as final order on June 21, 2019, with respect to the workplace located at 611 S; Linwood Road, Galesburg, IL 61401.
