On or about August 27, 2021 the employer at the USF Holland, LLC facility at 20820 Midstar Drive in Bowling Green, Ohio did not maintain an updated inventory of safety data sheets for chemicals used onsite as part of the hazard communication program for forklift operations for employees that transfer various flammable and corrosive chemicals on a daily basis. Products handled by employees contain hazardous chemicals including but not limited to methanol, magnesium sulfate monohydrate, 4-(2-nitrobutyl)morpholine, n-propyl bromide, and ethylene glycol.

29 CFR  1910.132(a): The employer failed to maintain provided protective equipment, including personal protective equipment for the eyes and extremities, in a sanitary and reliable condition where it is necessary by reason of hazards of processes and chemical hazards in a manner capable of causing injury or impairment.  On or about August 27, 2021 the USF Holland Facility at 20820 Midstar Drive in Bowling Green, Ohio did not maintain and store the goggles and gloves used for the changing out of the propane tanks on the forklifts in a sanitary condition. The personal protective equipment was stored outside and exposed to the weather and dust.
