29 CFR 1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: a) USF Holland, LLC at the location of 20820 Midstar Drive, Bowling Green, Ohio: The employer did not ensure that each operator had successfully completed the training prior to permitting employees to operate the propane powered forklifts. Sixty employees were tasked with operating forklifts as a part of their daily job duties however, the employer did not adequately provide formal instruction or certification for all employees prior to operating the propane powered forklifts, exposing employees to potential struck by hazards.
