29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: 8502 Mid County Industrial Drive: Employer is exposing employees to amputation hazards where the employees are permitted to use floor fans which are not adequately guarded. On and about 8/27/2021, employees in the production area were exposed to amputation hazards where the guard openings on floor fans were larger than 1/2 inch. The blade peripheries of the fans were less than 4 feet above the workplace floor.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: 8502 Mid County Industrial Drive, St. Louis, MO 63114, Line 1: Employer is exposing employees to electrical hazards where the employees are permitted to use equipment powered by flexible cord being used as a substitute for fixed wiring. On and about 08/27/21, employees on Liquid Line 1 were exposed to electrical hazards where a flexible cord set was used to supply power to a Belkin relocatable power strip which was in turn used to supply power to electrical equipment such as, but not limited to, an electric scale thereby using the flexible cord set as a substitute for fixed wiring.
