29 CFR 1910.212(a)(2): Guards were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: On June 29, 2023, an employee of Cerro Flow Products LLC was working on Furnace #1 conveyor at a building located at 101 South Douglas Avenue in Shelbina, Missouri. The employer did not ensure that a guard on Furnace #1 conveyor was securely affixed. An employee sustained amputations on two fingertips of the right hand while working near the unsecure guard.
