29 CFR  1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors:   a.  Basement Southside Exit at the top of the stairs: The exit door was locked at the door handle preventing quick egress.  Employees were exposed to smoke inhalation hazards in the event of a fire.  b.  Basement Northside Exit: The exit door was bolted at the bottom preventing quick egress.  Employees were exposed to smoke inhalation hazards in the event of a fire.

29 CFR  1910.1200(g)(1):  Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use:   Maintenance department: The employer had a written hazard communication program that did not include Safety Data Sheets (SDS's) for all hazardous chemical products used by the  maintenance department. Employees are exposed to physical and health hazards associated with the use of flammable paints, thinners, Quikrete, and caustic HVAC system cleaners such as Alcafoam.
