29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 425 South Salem Church Road, York, PA 17408, there was not an energy control procedure to address hazardous energy sources during the service and maintenance of conveyer belt M4 thereby exposing employees to a caught between hazard, on or about August 24, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: 425 South Salem Church Road, York, PA 17408, employees performing service and maintenance on conveyer belt M4 did not verify energy isolation/deenergization of the dollies exposing employees to a caught between hazard, on or about August 24, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
