The Hexamine Area - On or about February 28, 2025, employees were cutting open and dumping bags of hexamine a highly flammable powder into the dump station hopper without wearing flame resistant garments, employees were exposed to flash fire, or fireball hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The Hexamine Area - On or about February 28, 2025, employees were performing refractory operations loading hexamine into the hopper which used and generated combustible dusts, the control panel for the dust hopper was not installed as class 2, dust tight electrical, exposing employees to fire and explosion hazards. b. The Hexamine Area - On or about February 28, 2025, employees were performing refractory operations loading hexamine into the hopper which used and generated combustible dusts, rigid metal conduit was not installed as class 2, dust tight electrical, exposing employees to fire and explosion hazards. c. The Hexamine Area - On or about February 28, 2025, employees were performing refractory operations loading hexamine into the hopper which used and generated combustible dusts, the knife switch disconnect box for the fan motor was not installed as class 2, dust tight electrical, exposing employees to fire and explosion hazards. d. The Hexamine Area - On or about February 28, 2025, employees were performing refractory operations loading hexamine into the hopper which used and generated combustible dusts, the knife switch disconnect box for the class 2 electrical conduit running from the motor to the control of the hopper was not hermetically sealed above and below the equipment in two locations threaded openings were left open to allow dust entry into the conduit which was not installed as class 2, dust tight electrical, exposing employees to fire and explosion hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
