29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Facility wide - On or about September 2, 2021, the employer did not ensure that employees who operate powered industrial trucks, including Toyota 8FGU30 and 8FGU32 internal combustion counterbalanced forklifts, were properly trained, evaluated and certified to perform tasks in a safe manner, thereby exposing employees to struck-by hazards. (b) Facility wide - On or about September 2, 2021, the employer did not ensure that employees who operate powered industrial trucks, including a Caterpillar 262D skid steer loader, were properly trained, evaluated and certified to perform tasks in a safe manner, thereby exposing employees to struck-by hazards.

The extension cord was extended from a receptacle outlet on one side of an interior entryway to the opposite side in order to supply power to a portable Hitachi circular saw. b) Northeast building - On or about August 16, 2021, the employer failed to ensure that extension cords were not used as a substitute for fixed wiring. The extension cords were extended from a receptacle outlet in the main building to the Northeast building to supply power to a radio during plywood removal.
